This handbook provides in its 1078 pages an enormous amount of information on causes of accidents and on solutions, based on the well-known Engineering-Education-Enforcement trio. The authors have done a lot of work going through all the literature you otherwise would have to collect and read yourself.
And that's just why it's such a pity that there is no subject index. Sometimes it is a bit frustrating to know that this book probably contains what you are looking for, but that it takes such a long time to find it!
